Murphy Medical Center presents safety checklist for your medicine cabinets In an effort to make your healthcare experience a safe one, Murphy Medical Center would like to provide you with a Checklist for Medication Safety. During admission to Murphy Medical Center, all patients will be questioned closely about all medications that they are currently taking. Murphy Medical Center is providing free cards that fit easily into a wallet, providing a place to list all medications, as well as other important information, such as vaccination dates and known allergies. These free medication cards may be requested by calling the Public Relations Department at 835-5253 or the Patient Safety Department at 835-7519. Also, you may ask for these cards if you visit MMC Emergency Department, Urgent Care, or if you are a patient in the hospital. Unfortunately, medication errors happen. They happen in hospitals, in pharmacies, or even at home. Sometimes, people get hurt because of these errors. Your doctors, nurses and pharmacists work hard to keep you healthy, but you are also responsible. This checklist can help you make your healthcare experience a safe one. + Make a list of your medications. Make sure that all of your doctors know everything you are taking. This includes prescription and over-the-counter medicines, and dietary supplements such as vitamins and herbs. At least once a year, bring all of your medicines and supplements with you to your doctor. "Brown bagging" your medicines can help you and your doctor talk about them and find out if there are any problems. It can also help your doctor keep your records up to date, which can help you receive better quality care. + Make sure your doctor knows about any allergies and adverse reactions you have had to medicines. + Any time that your medications change, change your list, too. Double-check the imprints on the tablets and capsules. + Keep medications in their original containers. Many pills look alike, so by keeping them in their original containers, you will know which is which and how to take them. + Take your medications and the list of your medications with you when you go to the hospital. Your doctors and nurses will need to know what you are taking.
+ This list should include each medication name, strength, the number of pills taken and how often it is taken. Herbals and over the counter medications should also be included.
